Reporting an Incident or Accident on Site

1. When to Report
All incidents and injuries must be reported immediately to a member of Coinford site management — no matter how minor they may seem.
This includes:
• Injuries requiring first aid
• Near misses
• Damage to property, tools, equipment, or materials
• Any event that had potential to cause harm
• Illness or symptoms that develop during work activities
Delays in reporting can prevent Coinford from providing proper support and may impact investigations, corrective actions, and legal compliance.
2. Who to Inform
After informing site management, the following steps must be taken immediately:
1. Site management must phone the Health & Safety Department as soon as they are notified of an accident or incident.
2. The Contracts Manager must also be informed by phone.
3. If the usual H&S representative cannot be reached, contact another member of the H&S Department without delay.
Prompt communication ensures the correct level of support and a timely response.
3. Recording the Incident
All accidents — including minor injuries — must be entered into the site accident book on the same day
the incident occurs. If an operative reports an incident after the day it occurred, site management must:
• Contact the H&S Department before completing any accident book entry, and
• Send the completed entry to the H&S Department once agreed and finalised.
This ensures accuracy and prevents misunderstanding or mismanagement of the situation.
4. Why Immediate Reporting Matters
Recent incidents have shown that delays in reporting have led to:
• Missed opportunities for medical or welfare support
• Incomplete or delayed investigations
• Reduced ability for Coinford to provide assistance during hospital visits or time off work
Following the reporting procedure protects everyone and ensures any required corrective actions are taken quickly and effectively.
