Report an Incident
Site Observation Reports help everyone on site — operatives, subcontractors, supervisors, and management — identify good practice, highlight concerns, and prevent incidents before they happen.
To keep the site safe, we rely on everyone to report anything that could affect health and safety.
A Site Observation Report is a quick and simple way to alert site management to issues or to record positive behaviours that reinforce strong safety performance.
You can access the FlowForma Site Observation Report using the QR code on the site notice board, the FlowForma App, or the H&S Portal.
When to Report
Submit a Site Observation Report whenever you notice:
• Unsafe conditions – poor housekeeping, damaged equipment, missing barriers
• Hazards – anything that could cause harm if not addressed
• Unsafe behaviour – incorrect PPE, unsafe working methods
• Positive safety actions – tidy work areas, safe lifting, good teamwork
• Ideas for improvement
If you are unsure whether something needs reporting — report it anyway.
How to Report
1. Open the FlowForma Site Observation Report using the QR code or link.
2. Enter the Site Job Number and location of the observation.
3. Select the observation type: Near Miss, Good Practice, H&S Improvement, Unsafe Practice, or Hazard.
4. Describe clearly and factually what you saw, including any people involved (if known).
- Note any immediate action you took, if it was safe to do so.
5. Upload any relevant photos.
6. Submit the report — it will be sent to the Health & Safety Department and added to the Site Observation Report list.
Site management will review the observation, take appropriate actions, and provide feedback where required.
