Two Service strikes in 48 hours - Site stand down

Coinford sites have reported two service strikes in 48 hours - one LV main and one 63mm water main - the investigations are ongoing and will detail the failures once completed. However, these strikes show that some sites or people are becoming complacent to the safe procedure for working and excavating around buried services. Therefore we need all sites to complete a detailed TBT first thing tomorrow morning before works commence on site. The following must be strongly emphasised:

  • No excavation must commence until a Permit to break ground is completed, briefed and signed by all involved in the task. No Permit - no dig.

  • CAT and Genny survey. We are not completing thorough CAT and Genny surveys. We are not depth reading electric cables and this is contributing to striking cables shallower then expected. Trained operatives must ensure all abilities of the CAT and Genny are used. Remember the CAT detectors are logging how they are being used!

  • Do not use an excavator bucket within 500mm of a service. Using the excavator and hoping to see either the warning tape or sand before the bucket strikes the service will result in services being struck. If the ground is too hard to hand dig, stop and discuss a plan with the site supervisor.

  • Do not allow yourselves to be pressured into working faster or cutting corners to get the job done.

  • Ensure the full process as detailed in the Working around buried services RAMS and video presentation is fully adhered to.

Ensure a standdown is completed first thing in the morning and a thorough briefing is completed. Supervisors and managers to monitor thoroughly compliance going forward. Works around live services must become a main focal point.
